Die Behandlung der Influenza bei Personen, die nicht zu den Risikogruppen gehören und bei denen somit ein unkomplizierter Verlauf erwartet werden kann, erfolgt überwiegend symptomatisch. Eine spezielle Therapie mit antiviralen Arzneimitteln ist insbesondere bei Personen mit einem Risiko für Komplikationen zu empfehlen. Oft kommt es zu bakterieller Superinfektion, diese werden mit Antibiotika behandelt.

Zur Bekämpfung von Influenza Viren stehen drei Medikamente zur Verfügung: Amantadin, Oseltamivir und Zanamivir. Allerdings sollten beide Präparate spätestens 48 Stunden nach Einsetzen der Symptome gegeben werden. Amantadin hemmt das virale Membranprotein und damit das Eindringen des Virus in den Zellkern. Amantadin ist leider nur gegen Influenza A Viren wirksam und führt schnell zur Bildung resistenter Viren. Außerdem werden häufig neurologische Nebenwirkungen beobachtet.

Die Neuraminidasehemmer Oseltamivir und Zanamivir – besser bekannt unter dem Handelsnamen Tamiflu – blockieren die Aktivität der viralen Neuraminidase und damit die Freisetzung neu gebildeter Viren. Sie wirken auch beim neuen Influenza-H1N1-Virus. Nebenwirkungen sind auf Übelkeit und Erbrechen beschränkt. Aber beachten Sie, dass Oseltamivir nicht die Grippeschutzimpfung oder entsprechende Hygienemaßnahmen ersetzten kann.

Impfen Über das Pro und Contra bezüglich des Impfens bei Influenza wird heiß diskutiert. Die WHO hat eine Impfempfehlung herausgegeben. Ausschlaggebender Punkt ist auch, dass der Impfstoff nicht ausreicht, um die gesamte Bevölkerung zu immunisieren. Prinzipiell versichert aber die Regierung, dass sich jeder impfen lassen kann, der es will. Für Deutschland gilt aber folgende Impfreihenfolge: Als erstes werden Mitarbeitern im Gesundheitswesen und öffentlichen Dienst geimpft. Diese gelten nämlich als besonders gefährdet. Dazu gehören auch Beschäftigte in Pflegeeinrichtungen, Apotheken und dem Rettungsdienst. Ebenfalls unter den Ersten erhalten die Feuerwehr und die Polizei einen Impfschutz. Des weitern sollen Schwangere geimpft werden, da für diese die Gefahr groß ist bei Infektion mit H1N1 das Ungeborene zu verlieren. Schwangere werden aber nur geimpft, wenn der Impfstoff zweifelsfrei ohne Risiko für sie ist. Das wird gegenwärtig noch geklärt. Außerdem sollen bevorzugt Menschen mit chronischen Erkrankungen immunisiert werden, da auch sie besonders gefährdet sind bei Infektion mit dem H1N1 Virus zu versterben. Weiter werden dann alle gesunden im Alter zwischen 15 bis 49 Jahren, Kinder unter 15 und abschließend ältere Mitbürger über 50 geimpft.

Deutschland kann etwa ab September beginnen die Bevölkerung mit dem Impfstoff des Pharmaunternehmens GlaxoSmithKline zu immunisieren. Um einen ausreichenden Impfschutz zu bekommen, muss zweimal eine Injektion verabreicht werden. Wo man sich impfen lassen kann hängt vom Bundesland, in dem man lebt, ab. Informieren Sie sich genauer bei Ihrem ansässigen Gesundheitsamt. Beachten Sie auch offizielle Impfaufrufe in der Presse und den öffentliche Medien.
Die Kosten für eine Impfung betragen ca. 30 Euro und werden von den Krankenkassen und dem Bund/Staat getragen.

Bevor der Impfstoff im September der Öffentlichkeit zur Verfügung steht, wird er natürlich auf Nebenwirkungen ausgetestet. Schutz In diesem Kapitel erhalten Sie einige wertvolle Tipps und Verhaltensregeln, die Ihnen helfen sich vor einer Infektion mit Influenzaviren zu schützen.

Waschen Sie sich regelmäßig die Hände! Das regelmäßige Händewaschen sollte auch in grippefreien Zeiten eine hygienische Grundmaßnahme sein. Die Hände kommen den ganzen Tag über mit Gegenständen in Kontakt – und deshalb auch mit allerlei Krankheitserregern. Leicht dringen diese über die Schleimhäute der Augen, der Nase und des Mundes in den Körper ein. Wie oft langt man sich kurz ins Gesicht oder knabbert unbewusst an den Fingern? Doch wie wäscht man sich richtig die Hände? Halten Sie die Hände unter fließendes Wasser und reiben Sie sie 30 Sekunden mit Seife ein. Auch zwischen den Fingern! Anschließend spülen Sie sie sorgfältig ab und trocknen sie. Allgemein gilt, dass Flüssigseifen im Spender hygienischer sind.
Achten Sie immer darauf beim Husten Ihr Gegenüber nicht direkt anzuhusten, denn dabei schleudern Sie eventuelle Viren meterweit herum. Früher hieß es beim Husten Hand vor dem Mund halten! Das ist nur bedingt zu empfehlen – dabei bleibt eine große Anzahl von Viren aus Ihrem Körper an den Händen kleben und durch Berührung geben Sie diese weiter. Husten Sie also lieber nicht in Ihre Hände, sondern in den Ärmel. Dann bleiben Ihre Hände sauber. Versuchen Sie sich außerdem beim Husten immer von Ihrem Gegenüber abzuwenden.
Beim Naseputzen sollten Sie ausschließlich ein Einmaltaschentuch verwenden und dieses wirklich nur einmal benutzen und dann sofort wegwerfen. Waschen Sie sich im Anschluss immer gründlich die Hände!
Um die Verbreitung der Influenza zu verhindern, ist es besonders wichtig, dass Sie zu Hause bleiben wenn Sie erste Krankheitsanzeichen verspüren! Schleppen Sie sich nicht krank in die Arbeit! Besonders wichtig ist, dass Sie aufmerksam auf erste Krankheitsanzeichen achten. Typische Symptome für Grippe sind plötzliches hohes Fieber, schweres Krankheitsgefühl, Husten und Gliederschmerzen. Gönnen Sie sich Ruhe und geben Sie Ihrem Körper die Möglichkeit die Viren abzuwehren. Vergessen Sie nicht ausreichend zu trinken!
Gerade bei der Influenza reicht es nicht aus nur zu ruhen, viel zu trinken und sich auf Hausmittel zu verlassen! Kontaktieren Sie unbedingt Ihren Arzt, dies gilt besonders für sehr junge, sehr alte und chronisch kranke oder immungeschwächte Menschen.
Da wir es momentan mit einer H1N1-Pandemie zu tun haben, ist generell von großen Menschenansammlungen abzuraten. Aktuelles Wenn die kalte Jahreszeit kommt, dann ist auch wieder Grippezeit! Wie viele Menschen daran erkranken, ist jedoch von Jahr zu Jahr sehr verschieden und man bezeichnet diese Grippewelle gerne auch als saisonale Grippe. Im Extremfall kann sich aus der saisonale Grippewelle, oder auch unabhängig von dieser, wie es gerade der Fall ist, eine weltumspannende Pandemie entwickeln.

Wie bereits erwähnt, besitzen Viren eine Hülle, auf der sich verschiedene Oberflächenmoleküle befinden. Influenzaviren können sich, anders als Bakterien, aber nicht eigenständig vermehren, sondern müssen dazu in eine Wirtszelle eindringen. Beim Anlagern und Eindringen hilft ihnen das Hämagglutinin. Nachdem sie ihr Erbgut in die Wirtszelle eingeschleust haben, nutzen sie deren Zellsystem, um sich zu vermehren. Sind ausreichend Viren gebildet, verlassen sie mit Hilfe der Neuraminidase die Wirtszelle und befallen neue Zellen – das Virus breitet sich aus.

Die momentan sich ausbreitende Neue Grippe (Influenza-Virustyp A/H1N1) trägt nach Einschätzung der Weltgesundheitsorganisation (WHO) das Potential zu einer weltweiten Pandemie in sich. Erstmals ist der H1N1-Erreger in Mexiko aufgetreten. Es handelt sich hier um einen neu gemischten Influenzavirus, der seine Virulenzmerkmale aus Vögeln, Schweinen und dem Menschen (= Reassortment) selbst erworben hat. H1N1 wurde erstmals bei Schweinen nachgewiesen, ist aber für diese Tiere nicht gefährlich und verläuft auch nicht tödlich.

Das Gefährliche ist, dass der nun aufgetauchte Subtyp A H1N1 sich weiter entwickelt hat und nun auch direkt von Mensch zu Mensch übertragen werden kann.

Infiziert wird man, indem das Virus über die Schleimhaut der Atemwege, des Munds und der Augen in den Körper eindringt. Dazu reichen schon winzige Tröpfchen, die etwa beim Niesen, Husten oder Sprechen von Infizierten verbreitet werden. Allgemeines Die Influenza wird gerne als Grippe bezeichnet, obwohl es sich hier nicht um eine Erkältung oder viralen Infekt handelt, sondern um eine echte schwere Infektionskrankheit der Atemwege. Nach Schätzungen der WHO erkranken jährlich 10–20% der Weltbevölkerung an Influenza. Die Influenzaviren gehören der Familie der Orthomyxoviridaen an. Man unterscheidet Influenza A, B und C Viren. Der A-Typ kommt bei Mensch und Tier vor, wohingegen Influenza B und C ausschließlich im Menschen gefunden werden.

Auslöser der großen Epidemien und Pandemien sind meist Influenza A Viren, da diese in der Lage sind, ihre Oberflächenmoleküle Hämagglutinin (HA) und Neuraminidase (NA) ständig zu verändern. Das Influenzavirus besitzt ein Hülle, auf der sich kleine spikeartige Fortsätze, die Oberflächenmolküle NA und HA, befinden, die Hülle selber ist aus Matrixproteinen und Lipoproteinen aufgebaut. Es gibt mittlerweile 16 verschiedene HA und 9 NA-Typen. Die Nomenklatur der Influenza-Viren erfolgt nach dem Typ und Subtyp, wie zum Beispiel Influenza A – H1N1.
